Steps to take
A business would notify us they would like to learn about the free Five Pillar Wellness Risk Management
program. Free for businesses.
- No cost to a business to enrol or to participate in this research
- We set up an interview to discuss our program.
Interview Itinerary
The business would agree to notify their workers that the Five Pillar Wellness Risk Research Program is available
to them. Basically, this is the only business' obligation.
- Our Wellness goal is to address the action plan end of Wellness.
- Our Wellness model is the Five Pillars

- What the individual will receive by way of a Wellness Progress Chart
- What the business will receive by way of aggregate Wellness numbers for each cohort

- Business Choices

- Business decides what research cohorts it wishes to establish. For instance, Management, Staff,
Workers, Separate geographic locations etc.
- Business notifies its management and work force about our program

- This is where some cost might be found, but most companies can do this notification at no cost or
little cost using it's present workforce contact list an d procedures.
- An interested worker enrols in the voluntary, anonymous research program and agrees to the protocol.
- The worker completes the three online protocol forms. Rough time commitment to complete the forms,

- Form #1 takes about 10 minutes via a PDF fillable guess form sent via email.
- Worker waits for their enrollment email from us.
- When enrollment is received by the worker they progress to Form #2.
- Form #2 the Questionnaire online, takes about an hour sometimes more, if one knows English well. It
takes less time than watching a movie.
- The worker receives their Wellness Progress Chart with their 38 numbers automatically via email
after completing Form #2.
- Form #3 takes 5 minutes via a PDF fillable form in an email.
- The worker agrees to complete this process several times.

- The business receives its business report when sufficient statistics have been collected. We like a minimum
of 10.
Optional Choices
- The worker decides if they wish to enrol in an optional Five Pillar Wellness Workshop.
- The worker decides if they are interested in the optional FoodPainConnection program including "Dr.
Ouellette's Anti-Inflammatory, Anti-Obesity, Anti-Aging, Pain Relief, Wellness Diet" exclusively with Dr.
Ouellette, the originator and creator of that dietary program.
Five Pillar Wellness Workshop Fees
- Wellness workshop fees are on a per hour basis, are covered by the worker and are designed to troubleshoot
issues a worker may have with their Five Pillar Wellness Risk Action Plan. We have a special reduced Inauguration
Research Rate.
- No actual health advice is given in the Five Pillar Workshop program unless the Pain Relief Diet option is
chosen wherein the advice relates to implementing the diet plan. Two workbooks by Dr. Ouellette on the
Anti-Inflammatory Pain Relief Diet are available online.
- The workshop is all about completing the Wellness Risk Action Plan list. The
Wellness Action Plan List is built by a computer algorithm from the answers to the five pillar questions and is
delivered in a Wellness Risk Report of
Findings provided as part of the Workshop.
- The research program builds numbers for both the individual and the business as aggregate cohort statistics.
The business does not receive an individual's Wellness Risk scores.
- Nutrition, Exercise, Spinal Health, Rest & Relaxation, Attitude & Perspective in life all have a very
good science base to qualify them as Wellness Pillars.

We would be happy to arrange a lecture time to come to the business and discuss our program with management and/or
workers.
